ABSTRACT

Objective@#To observe the clinical effects of stage-Ⅱ Meek skin grafting on adipose tissue after tangential excision in patients with extensive deep burns, and to explore the functional mechanism.@*Methods@#The medical records of 26 extensively burned patients who met the inclusion criteria and were admitted to the Department of Burns and Plastic Surgery of the Fourth Medical Center of PLA General Hospital from May 2015 to December 2017 were retrospectively analyzed. According to the treatment methods, 14 patients were enrolled in stage-Ⅰ skin grafting group (10 males and 4 females, aged 27 to 75 years), and 12 patients were enrolled in stage-Ⅱ skin grafting group (10 males and 2 females, aged 31 to 76 years). Patients in the 2 groups all underwent debridement of tangential excision, and their healthy adipose tissue was preserved. Meek skin grafting was performed just after tangential excision in patients in stage-Ⅰ skin grafting group. In patients in stage-Ⅱ skin grafting group, porcine acellular dermal matrix (ADM) was applied to cover the wound after tangential excision, and 3 days later, it was removed and Meek skin grafting was performed. The times of complement skin grafting and the wound basic healing time of patients in the 2 groups were observed and recorded. In the stage-Ⅱ skin grafting group, the adipose tissue of patients were taken from the wound center immediately after tangential excision and immediately after the removal of porcine ADM, for the observation of structure of the fault surface of adipose tissue through hematoxylin and eosin staining and microvessel density (MVD) through immunohistochemical staining. Data were processed with independent sample t test and Fisher′s exact probability test.@*Results@#(1) The times of complement skin grafting of patients in stage-Ⅱ skin grafting group was (1.83±0.17) times, which was obviously less than (3.36±0.63) times in stage-Ⅰ skin grafting group (t=2.19, P<0.05). The wound basic healing time of patients in stage-Ⅱ skin grafting group was (35.1±2.3) d, which was obviously shorter than (48.8±4.9) d in stage-Ⅰ skin grafting group (t=2.27, P<0.05). (2) Immediately after tangential excision, the intercellular substance was few between the adipose cells in adipose tissue of patients in stage-Ⅱ skin grafting group. Immediately after the removal of porcine ADM, there was regenerated granulation tissue in the intercellular space of adipose cells of adipose tissue of patients in stage-Ⅱ skin grafting group. Immediately after tangential excision, the MVD of adipose tissue of patients in stage-Ⅱ skin grafting group was 20.2±1.3 under per 400-time field, which was obviously less than 32.2±1.9 under per 400-time field immediately after the removal of porcine ADM (t=-5.38, P<0.01).@*Conclusions@#Meek skin grafting on the adipose tissue in stage-Ⅱ surgery after tangential excision could reduce the times of complement skin grafting and shorten wound healing time of patients with extensive deep burns. The mechanism may be related to the improvement of the recipient condition of adipose tissue.

ABSTRACT

Objective To explore the basic functional orientation of countywide healthcare alliances and how to maintain their stable operation and sustainable development direction. Methods 42 government officials, experts, employees of countywide healthcare alliances and residents were interviewed. The grounded theory was used to analyze data from the interview, with the core categories gradually extracted. Results Three basic functions were defined for such healthcare alliances, namely basic functions, stable operation and sustainable development. Conclusions The healthcare alliances prove an effective way to build the hierarchical medical system and improve people's health in general. Such alliances rely on good governance mechanism, reasonable balance mechanism of interests and scientific resource sharing mechanism.

ABSTRACT

Objective To investigate the expression changes of acetylcholinesterase (AChE) related microRNAs derived from peripheral blood mononuclear cells (PBMCs) in patients with stroke. Methods The microRNAs for targeting AChE mRNA were selected via prediction software and previous studies. PBMCs were extracted from venous blood samples of acute ischemic stroke patients (onset<24 h) and healthy controls. The expressions of microRNAs and AChE mRNA were quantified using real-time PCR (RT-PCR). The protein level of AChE was detected by Western blot assay. Results Thepredicted microRNAs included microRNA (miR)-24,-28,-124,-132,-182*,-194 and-484. The expression levels of miR-24,-124,-132 and-194 were significantly elevated in stroke patients compared with those of controls (P<0.05). There were no significant changes in expression levels of miR-28,-182*and-484. Additionally, the relative expression levels of intracellular AChE mRNA and protein decreased significantly in stroke patients (P<0.05). Conclusion MiRNAs can enhance cholinergic anti-inflammatory pathway by targeting AChE in patients with acute ischemic stroke.

It emphasized the establishment of clinical thinking in medical records and divergent thinking extended from clinical thinking. Through the analysis of the typical medical record, this article hoped to provide references for the comprehensive study and application of febrile diseases, golden chamber, and warm diseases.

ABSTRACT

PurposeTo explore the significance of dual energy CT in differential diagnosis of squamous cell carcinoma metastatic lymph nodes and lymph node tuberculosis in the neck.Material and Methods Twenty-five patients with a total of 62 cervical enlarged lymph nodes confirmed with squamous cell carcinoma metastasis or lymph node tuberculosis by pathology underwent dual energy enhanced CT scan. Iodine coverage value of two different kinds of pathological properties of lymph nodes was measured. The trend of the spectrum curve was observed and the energy spectrum curve slope of two kinds of lymph nodes were compared. The sensitivity, specificity and accuracy of energy spectrum curve slope in differential diagnosis were analyzed. Results Of all 62 lymph nodes, there were 32 squamous cell carcinoma metastatic lymph nodes and 30 lymph node tuberculosis. There were statistic significance in the iodine coverage between the squamous cell carcinoma metastatic lymph nodes and lymph node tuberculosis (t=3.806 and 3.698,P<0.05). In the range from 60 keV to 180 keV, CT value of the two kinds of lymph nodes decreased with increase of keV value. It showed the higher the keV value the less CT value decreased, with the energy spectrum curve of drop type. The area of the iodine coverage was 0.756 with the differential diagnostic sensitivity of 56% and the specificity of 80%. The area of energy spectrum curve slope was 0.898 with the differential diagnostic sensitivity of 76% and the specificity of 85%.Conclusion The iodine coverage value and the energy spectrum curve slope have significance in differential diagnosis of the cervical squamous cell carcinoma metastatic lymph nodes and lymph node tuberculosis. The energy spectrum curve slope is superior to the iodine coverage value in differential diagnosis.

ABSTRACT

Objective To e xplore the clinical value of adrenomedullin on children with acute heart failure treatment and progno-sis.Methods Selected 58 cases of AHF children,40 patients with pneumonia,40 cases of children with congenital heart disease and 50 healthy children as the object,radioimmunoassay was taken to detected plasma adrenomedullin and endothelin level,and analysed correlation between AMD,ET-1 and cardiac function.Results Pneumonia complicated with heart failure group,congenital heart disease complicated with heart failure group plasma AMD,ET-1 levels were significantly higher than those in the control group, congenital heart disease group and pneumonia group (P<0.05),congenital heart disease group and pneumonia group of plasma AMD,ET-1 levels were significantly higher than those in the control group (P<0.05).Heart failure stage plasma AMD,ET-1 lev-els were significantly higher than those of early stage and recovery period (P<0.05),heart failure early stage plasma AMD,ET-1 levels were significantly higher than those of early heart failure recovery period (P<0.05).There was a significantly negative cor-relation between with AMD,EF-1 levels and CI,LVEF in AHF children (r= -0.614,-0.793,-0.618,-0.547,P<0.05);AMD mutation rate was significantly higher than that of EF-1 (P<0.05).Conclusion Plasma levels of ADM are closely relate with heart function in AHF children,through the determination of plasma ADM level can assessment treatment and the prognosis of children with AHF,and worthy of clinical application.

ABSTRACT

Objective To detect the transfection of pIRES 2-EGFP-NT3 in mouse cochlea fibroblast by cationic liposome . Methods After pIRES2-EGFP-NT3 had been abstracted successfully , it was transfected into mouse cochlea fibroblast by lipofectami-neTM2000.Twenty four hours later, the efficiency of the transfection was analyzed by confocal microscope .Results The pIRES2-EG-FP-NT3 was effectively transfected into mouse cochlea fibroblast by cationic liposome .The transfected fibroblasts displaying green fluo-rescence were observed under fluorescence microscope .Conclusions The effective transfection of pIRES 2-EGFP-NT3 into mouse cochlea fibroblast by lipofectamine TM 2000 laid the basis for the following experiments , such as NT3 gene transfection in deaf or normal cochlea and so on .

ABSTRACT

OBJECTIVE@#To evaluate the clinical efficacy of the intratympanic methylprednisolone perfusion for patients of intractable Meniere's disease (MD).@*METHOD@#Ten cases (10 ears), collected from Janu 2008 to Janu 2010, of intractable MD were studied retrospectively. The micro-catheter was placed into the tympanum of the affected ear, then methylprednisolone was imported into the tympanum once a day for 10 days. The average followed-up duration was (15. 4 +/- 5. 4) months after the perfusion. The treatment effect of vertigo, hearing loss and activity capacity was evaluated with Diagnostic Criteria for MD set by the Guiyang Meeting in 2006. The efficacy of tinnitus was analyzed by comparing the score of tinnitus handicap inventory (THI) before and after treatment.@*RESULT@#The vertigo was absolutely controlled in 7 patients (grade A), partially controlled in 2 patients (grade B) and no efficiency in 1 patient (grade C). The 2 patients in grade B had a vertigo again in 9 months and 11 months after intratympanic perfusion, respectively, while the frequency, severity and duration of their vertigo softened obviously. Puretone threshold average (PTA) at the affected frequencies was decreased to within the level of 20 dB in one patient and was improved more than 30 dB in another one (grade A), improved 15 dB to 30 dB in 4 patients (grade B), and improved less than 15 dB in the other 4 patients. The average score of THI was 48.80 +/- 7.25 and 41.9 +/- 7.78 before and after perfusion respectively. The ability capacity of all the 10 patients after treatment was as normal (grade A), i. e. All of them could lead an independent life. There was no irreversible tympanic perforation in the operated ear and there was no other complications left.@*CONCLUSION@#Intratympanic methylprednisolone perfusion through the micro-catheter is a safe and effective method for the intractable Meniere's disease.

OBJECTIVE@#To study the bone conductive hearing loss in adults of otitis media with effusion (OME).@*METHOD@#Fifty adults of OME treated in our hospital, from Mar. 2009 to Feb. 2010, were enrolled for analyzing bone conduction hearing loss (BCHL) before auripuncture and after auripuncture, the difference between which were compared in 51 ears. BCT of pre auripuncture 24 subjects, BCT of post-auripuncture 22 subjects, and BCT recovered from OME 9 subjects compared with the contralateral ear respectively. High and extend high frequency (8, 10, 12, 16 kHz) air conduction threshold (ACT) after recover from OME was compared to the normal contralateral ear in 4 adults.@*RESULT@#BCT improved significantly after auripuncture at the frequencies (0.5-4.0 kHz), and the improvements in 4.0 kHz are more than that in 0.5 kHz significantly (P<0.05). In the unilateral OME subjects, BCT, of post auripuncture and recovered from OME, nearly recovered to the same level as the contralateral ear in most cases. While high and extend-high frequency (8, 10, 12, 16 kHz) ACT elevated in 3 of 4 patients recovered from OME.@*CONCLUSION@#Both effusion in middle ear and injury in inner ear could result in the elevation of BCT. Elevation of BCT at regular frequencies (0.5-4.0 kHz) is frequently associated with the effusion in middle ear, while these frequencies were insensitive in the early inner ear injury. The high and extend high frequency ACT elevation may be sensitive for the early sensorineural hearing loss. As the disease prolonged, sensorineural hearing loss of the lower frequencies (0.5-4.0 kHz) could be detected of OME patient.

OBJECTIVE@#To study the expression of vascular endothelial growth factor(VEGF) and microvessel density (MVD), in nasal polyps and its significance.@*METHOD@#The expression of VEGF, in nasal polyps from 50 patients and inferior turbinates from 10 patients were studied with immunohistochemical methods, measuring their grey score, the relations between expression microvessel density were analyzed.@*RESULT@#(1) The differences between the VEGF expression in nasal polyp gland and vascular were significant (P < 0.05). (2) The differences between the microvessel quantities in nasal polyps and inferior turbinates were significant (P < 0.05). (3) The association between VEGF expression in vascular of nasal polyps and microvessel density was significant (r = -0.664 (< 0. 05).@*CONCLUSION@#In nasal polyps, VEGF, expression increased. Those results show that the abnormal expression of VEGF and MVD may play an impotent role in the development of nasal polyps. There was an important value for proper treatment and prediction of nasal polyps.

ABSTRACT

OBJECTIVE@#To evaluate the efficacy and safety of implantable cardioverter defibrillator (ICD) and cardiac resynchronization therapy-defibrillators (CRT-D) avoiding defibrillation threshold (DFT) testing when treating ventricular tachycardia (VT) or ventricular fibrillation (VF).@*METHODS@#We analyzed a continuous database of the 21 patients who had avoided DFT during ICD implantation from Oct. 1999 to Aug. 2008. Follow-up data were completed and analyzed in the 21 patients with ICD implantation.@*RESULTS@#ICDs were implanted successfully in 17 patients with VT or VF, and CRT-D were implanted successfully in 4 myocardiopathy patients with severe heart failure who avoided DFT during ICD or CRT-D implantation. Eight patients accepted DFT 1 week later, VT or VF was not induced in 3 patients (37.5%). During the mean follow-up of 1 approximately 7 (4.2+/-1.9) years, malignant ventricular arrythmia was recorded in 16 patients. Among them, 89 episodes were successfully terminated by defbrillation (100%), 120 VT events were terminated by the first run of antitachycardia pacing (51.1%) and 22 by low energy cardioversion (59.2%). All patients took antiarrhycardia drugs after ICD or CRT-D implantation. No patient died from malignant ventricular arrythmia during the follow-up.@*CONCLUSION@#No application of routine DFT may avoid complications associated with DFT during ICD or CRT-D implantation. ICD or CRT-D implantation may effectively treat fatal ventricular tachyarrhythmias and prevent sudden cardiac death.

ABSTRACT

BACKGROUND: The cell growth, proliferation and differentiation caused by p38 mitogen-activated protein kinase (p38MAPK) might act as the common pathway in the onset and development of diabetic vascular complication.OBJECTIVE: To investigate the effect of atorvastatin on p38MAPK signal pathway and the influence of atorvastatin on cell proliferation and expression of transforming growth factor-β1 (TGF-β1) at transcriptional level in human glomerular mesangial cells (HGMCs) cultured with oxidative modification of low-density lipoprotein (ox-LDL).DESIGN: A randomized, parallelized, controlled and open trial.SETTING: Endocrinology Department, First Hospital Affiliated to China Medical University; Endocrinology Department,Respiratory Department, Urology Department, the General Hospital of Shenyang Military Area Command of Chinese PLA.MATERIALS: The experiment had been done in the laboratories for Pharmaceutical Department of China Medical University and Respiratory Department of Shenyang Military Area Command of Chinese PLA from May 2004 to May 2005. The sample was cut from renal cortex from the healthy segment of nephroectomy from a tumor patient (Provided by Xiang Jun, Urology Department, the General Hospital of Shenyang Military Area Command of Chinese PLA; Informed consent was obtained). OX-LDL was purchased from biochemistry institute of Peking Union Medical College (Batch No.20040711 ). ox-LDL was 5.3±1.0 nmol in 100 μg protein. Atorvastatin was purchased from Pfizer Pharmaceutical Co. Ltd (No. 45837088); p38MAPK monocloncal antibody was purchased from Santa Cruz.METHODS: ① 6.0-8.0 cm3 blocks of cortex were cut from renal cortex from the healthy segment of nephroectomy from a tumor patient, glomerular mesangial cells were isolated. When grew into 80% confluent monolayer, the cells were digested and performed passage. After the first two passages, the cells were pure based on morphology and characterized by DAB staining for Vimentin antigen and actin antigen was positive, whereas cytokeratin antigen was negative. Oil red "O" staining confirmed that ox-LDL was intaken by HGMCs. The 4-8th passages of cells were used to study. ②HGMCs were seeded into 96-well plates with 5×103 cells per well and grown in 200 μL culture medium. The study was divided into 5 groups (6 wells each group): 1.2, 6,12 mg/L atorvastatin group, ox-LDL group and blank control group. The cells were pre-incubated with atorvastatin for 30 minutes, then exposed to 80 mg/L ox-LDL. The cells in blank control group were untouched. After 24 hours, MTT was added. The absorbance of each sample at the wavelength of 492 nm was measured with immunosorbant assay system. The inhibitory rate of cell proliferation was calculated. ③1×106 to 3×106 cells were seeded into six 200 mL flasks. The trial was divided into 5 groups randomly:control group, 10, 40, 80 mg/L ox-LDL groups and atorvastatin group (12 mL/g). The cells in each group were pre-incubated for 30 minutes, then exposed to 80 mg/L ox-LDL for 24-routine culture. The expressions of TGF-β1mRNA of harvested cells were detected with semi-quantitative reverse transcription-polymerase chain reaction and p38MAPK signal pathway activation was detected by Western blot.MAIN OUTCOME MEASURES: ①Identification results of HGMCs. ② Proliferation of HGMCs. ③ TGF-β1 expression of HGMCs. ④p38MAPK signal pathway activation of HGMCs.RESULTS: ①When the cells were sub-cultured to the second generation, cell volume was big. Most of the cells were spindle-shaped, irregular stellate or branch-like, filled with microfilaments which paralleled axis. Cells overlapped in the intensive area. After DAB staining, cytoplastic actin and vimentin were positive and keratin was negative. Oil red "O"staining confirmed that ox-LDL was intaken by HGMCs with red granules in the cytoplasma, while control group did not.It was proved that the cells cultured for passage were HGMCs. ② As compared with control group, the inhibitory rate of cell proliferation in ox-LDL group was significantly decreased, but that in atorvastatin 1.2, 6 and 12 mg/L groups was significantly increased (0, -17.4%, 6.4%, 22.5%, 61.5%, respectively, P < 0.05 or 0.01) on concentration-dependent manner. ③ As compared with control group, ox-LDL (10, 40, 80 mg/L) increased the expression of TGF-β1 and activation of p38MAPK in concentration-dependent manner, the effect of 80 mg/L ox-LDL group was the most significantly (P < 0.01). Atorvastatin decreased the increment of TGF-β1 expression and the activation of p38MAPK pathway induced by ox-LDL significantly. There was significant difference when compared with 80 mg/L ox-LDL group (P < 0.01).CONCLUSION: Atorvastatin can antagonize the activation of p38MAPK pathway, decrease the secretion of TGF-β1 and inhibit mesangial cell proliferation induced by ox-LDL, suggesting that it may exert beneficial effect in the prevention and treatment of diabetic nephropathy with dyslipidemia.

ABSTRACT

OBJECTIVE To investigate the surgical methods for the lesions involved the common carotid artery.METHODS The clinical data of 11 cases with lesions involved the common carotid artery who underwent operations were retrospectively studied.The lesions were 1 case with recurrence tumor after 3/4 partial laryngectomy,1 case with bleeding of the carotid aneurysm caused by tuberculosis,1 case with iatrogenic carotid aneurysm,3 cases with carotid body tumor,1 case with thyroid gland cancer,2 cases with neck tumor,1 case with injury of the carotid artery and 1 case with gas gangrene.RESULTS Common carotid artery was reconstructed in 2 cases after removal of the tumors.The tumors were resected using the carotid shunt in 2 cases.Common carotid artery was sutured in 1 case with neck injury.The common carotid artery was repaired in 1 case with iatrogenic carotid aneurysm after removal of the tumor.The carotid artery was dissected out from the thyroid gland cancer in 1 case.The common carotid artery was reserved in 2 cases after resection of the neck tumors. Neck drainage was performed in the case with gas gangrene.CONCLUSION The surgical methods for lesions involved the carotid artery after removal of the tumors include the reconstruction of the carotid artery, resection and suture the carotid artery,and free of the carotid artery from the tumors.

Methods:8 patients with HA were involved in this study.All of them were studied by nuclear medicine (NM)with dual-radiopharmaceuticals.The 99 Tcm-SC imaging was perfomed first ,then the 99Tcm-EHIDA im aging two days later,and the ratios between lesions and normal liver tissue(L/N) were calculated in terms of different counts within the same size regions of int erest (ROI) in different phases(30min,1h and 2h respectively).Some of the cases were also analysed by CT and/or MRI within one week before the operations. Results:All of the cases were submitted to surgical treatment and c onfirmed diagnosis of HA by pathology. For HA, scintigrahy presented a accuracy of 100%(8/8),which was higher than that of CT and MRI comparatively.For 99 Tcm-EHIDA imaging,there were significant differences in L/N ratios among different phases(30min,1h and 2h). Conclusion:The results show that radionuclide imaging proved to be a safe,invasive and effcetive method on the diagnosis and differential diagnosis of HA,especially ,when CT and/or MRI findings are not typical.

ABSTRACT

Aim To investigate the effect of exposure to high or low concentration chemotherapeutic drugs on multidrug resistance of human breast cancer cell MDA-MB-231.Method MDA-MB-231 was treated with high dose of adriamycin and paclitaxel or with low concentration of paclitaxe.SRB assay was used to determine the IC50 and RF.HE assay was used to evaluate the cellular morphology.The variations of P-gp and MDR1 were analyzed by immunocytochemistry and RT-PCR respectively.Results Cells survived only after treated with high dose of paclitaxel (MDA-MB-231/a).SRB assay showed that the growth rate of MDA-MB-231/a was the same as that of parent MDA-MB-231/w.The IC50 of the cells(MDA-MB-231/b)treated with low concentration of paclitaxel to several chemotherapeutic drugs was a little higher than that of MDA-MB-231/w.Immunocytochemistry showed that there was no difference between MDA-MB-231/a and MDA-MB-231/w in the expression of P-gp while the P-gp expression was a little higher in MDA-MB-231/b.RT-PCR assay showed that the MDR1 gene was over-expressed in MDA-MB-231/b.Conclusions The multidrug resistance cell lines can not be derived from MDA-MB-231/w by high dose of chemotherapeutic drugs.Induction of multidrug resistance response to chemotherapeutic drugs is related with transient exposure to low concentration of paclitaxel and this may be a way to establish the multidrug resistance model of MDA-MB-231 cells.

ABSTRACT

Objective To study the chemical constituents of Astragalus membranaceus var. mongholicus. Methods The constituents were isolated and purified by several chromatographic techniques and identified by chemicophysical properties and spectral analyses. Results Nine flavonoid compounds had been obtained from A. membranaceus var. mongholicus. They were determined as formononetin (Ⅰ), (3R)-8, 2′-dihydroxy-7, 4′-dimethoxy-isoflavane (Ⅱ), calycosin (Ⅲ), (6aR, 11aR)9, 10-dimethoxypterocarpan-3-O-?-D-glucoside (Ⅳ), 7, 2′-dihydroxy-3′, 4′-dimethoxy-isoflavane-7-O-?-D-glucoside (Ⅴ), formononetin-7-O-?-D-glucoside (Ⅵ), calycosin-7-O-?-D-glucoside (Ⅶ), pratensein-7-O-?-D-glucoside (Ⅷ), and genistin (Ⅸ), respectively. Conclusion Compound Ⅷ is obtained from the plants of Astragalus Linn. for the first time and compound Ⅱ is obtained from this plant for the first time. Compounds Ⅰ-Ⅶ show cell multiplication activity.
